7. Complete Educational Records, Transcripts, Grades, Test Scores

8. Personal Recommendation Form

9. Recommendation by a Teacher of the Transcendental Meditation® program Form

10. Immunization Card and Certificate of Dental Screening or a Notarized Waiver for each

11. Recent Pupil Physical and Health Record, completed within the last 12 months

12. Interview with Principal, Admissions Director and/or Academic Counselor

Addendum for International Boarding applicants:

1. Confidential Financial Statement Form

2. Copy of Passport

3. SLEP and/or TOEFL test scores

4. Proof of Health Insurance
